NASCO Member Health Assessment Scheduling Form
This form will help us find a time that will work for your co-op to participate in a member health assessment interview!

Ideally, health assessment interviews include multiple participants and have representation of the general membership/residents, the board of directors, staff, and any other major stakeholders in the health of your co-op. We recognize that it can be difficult to get all of those people together to meet, and we can work to accommodate requests to meet during the work day, in the evenings, or on weekends as needed!

Health assessment interviews generally take about 2 hours, and include questions about your co-op's policies, financial status, the dissemination of information within your organization, cooperative culture, and a variety of other topics. You can find the assessment questions here.
